The message is a part of the form depicted in the linked image. Would putting the whole text into a single message (with multiple parameters replaced with HTML form elements) be the recommended approach here?

I think you did pretty well on the form. The problem here is that nothing of this is documented. Adding qqq with a brief explanation and the screenshot will help a lot.
