Non-working HTML code at Mwe-upwiz-tooltip-other
Mwe-upwiz-tooltip-other ("Any other information you want to include about this work.") tries to use HTML, but the code (see mw.UploadWizard.js, addHint() function) seems not to support that, which means the HTML code is shown to the user (you can test that at http://commons.prototype.wikimedia.org/wiki/Special:UploadWizard).
