Is this still the case? It seems like a Jira account is required before one can even see the form.

The new CLDR website and issue tracker often require JavaScript etc. where they're not supposed to. There are some issues filed about it. Having an account probably helps so I'll remove that sentence.

I'm not sure we've had any successful case of new locale creation in the past couple years, so I'm not sure what example to follow, but I can help whoever wants to try.
