I now see: "I, ____, the copyright holder of this work, irrevocably grant anyone the right to use this work under the Creative Commons Public Domain Dedication". As I noted a while ago, this text is incorrect - the CC PD dedication is a separate document from CC0 and not intended for licensing of one's own works. (The external link is correct, however). I'm not sure how to fix it. source

I would suggest reporting a bug for UploadWizard for this.
